No idea what model....it's probably in the 1400 series range.
Date-wise, it's probably from Eureka's "Brandy-Wine" era....late 1970's.

That is a kmart model with a 600 series model number (628 maybe). It should be a 4 amp motor and is from about 1983-1986. It's in the second series of Brandywine.
